By mail: To request a two-week loan of
one of the items above, attach your business card to a copy of this form (it isn't necessary to send the whole list, just the relevant page), clearly marking the book or video you wish to check out, and send your request with a $5.00 check made payable to South Carolina Bar at the address below:
